Non-Insured Services

Hawthorne Village Medical Clinic has available, as an option, an annual block fee. This fee replaces certain individually billable, non-insured, items or services such as medical leave notes, telephone refills and physician call-backs.

The annual fee at the clinic is $100 per patient and includes the following:

• Doctor’s notes (“sick notes”) – upto 10 per year.
• Telephone refills of prescription medicine
• Same-day telephone call back by a physician
• Email support
