Prescription Refill
When you need a prescription refilled, please have your pharmacy contact
us directly (either by fax of phone). With your doctor’s approval, we can coordinate the
refill for you, saving you time.
If you subscribe to the block fee, you can also call the clinic at (905) 878-0676. Your doctor will review your charts and call your pharmacist to refill your prescription if appropriate. We will need the following information:
- Your full name and phone number
- Prescription number
- Name and dose of the medication, with instructions
- The name and phone number of the pharmacy where you will obtain the medication
Your request will be confirmed by phone. If your prescription cannot be refilled, we will contact you by phone. Please allow a 24-hour turn around time for refills. If you have not had a patient visit in the clinic within the past 12 months, you will be advised to see your doctor.

HOW TO DETERMINE IF YOUR PRESCRIPTION IS STILL VALID
First, determine the number of refills left on the prescription (will usually be in the upper right corner of the label and will look something like REFILLS: 3 of 5). If the number of refills left is 0, then the prescription is no longer valid, and you should call the clinic or the pharmacy for a refill before the medication will run out.
WHAT DO I DO IF MY PRESCRIPTION IS NO LONGER VALID
If your prescription is more than one year old or has no refills left on it, it cannot be refilled and a prescription or your doctor authorization must be obtained.
If your prescription is no longer valid and you need more medication, you should call the Clinic at (905) 878-0676, between the hours of 9a.m and 8p.m.
